Survey Monkey Clone  

I need an experienced reseacher who can work with findings from a 20-question Survey Monkey study and present the data in a clean Excel spreadsheet, summarise the findings in English with basic Excel Graphs.

We are looking for someone who is experienced and savy with Survey Monkey to do a quick 1 day turn around of a 49 question survey that we have created. We would need it to have "skip" capability.

should not take you more than 30 minutes - 1 hour to implement because I have the code, I just need someone to write a quick JavaScript pop-up script. I would prefer that the script for the pop-up not be coded on the HTML page itself, but be called.

Furthermore, the exit-pop up will need to be installed on 3 pages on the website where the form appears.

Programmer MUST work in agile programming method, meaning I want continuous iterations (releases) of development, and I will pay a portion of the total payment after each iteration - until completion.

The changes I want to survey monkey are not drastic, but will be explained upon selecting the winner - therefore the person must be very professional.

There do not need to be any e-commerce related functions of the site - everything will be 100% free.

all their forms and I don't want to have that field. I also have used survey monkey but to imbed a small survey on my page seems very complicated.

I just need to gather the survey answers on an excel spread sheet. I am not very IT savy but I can put basic HTML codes on my web page and do very basis editing of HTML codes.

I need someone who can do this survey in a very simple way for me and tell me in non geek terms how to get it working on my site.

We require the responses to be emailed to a pre-defined email address.

The visual design is very important to us - which is why we arent using survey monkey but rather have had a Psd design created for each page.

Please do not respond if the html supplied will not look exactly like the psd supplied.
