10$ for ASP Form problem

I have a form(See attached file), It has some problem.
Form actually processes BUT the information is not coming through to the email. Knowledgable person can solve it within 10 minute, 10$ for 10 minute job.
Note: I need the code to be very well documented and complete this job within 1 to 2 hours of time.

Most of the form fields are working fine, but the "Date" dropdown menus are defaulting to "January 1" (regardless of what date the user enters). The rest of the fields are working fine.
The form script is "formadult1.asp" (attached).
The FLA file is also attached.
Please send a price quote for fixing the problem.

Pre populate of asp form
We have a small html form of about 8 to 10 fields. Once this form is submitted we wish the contents of the form to populated an asp form and, of course, to send us a copy of the contents of the first form.

I have an ASP form that users of my site use to register and upload small video files (wmp compatible) with, and i need need it checked out because the upload of file does seem to work, and I need it tested. This is a rush job for right now.

ASP Form and MS SQL Database by webgalis
Simple project. Need ASP form with 9 buttons and 6 subcats for each of the 9 buttons. Also going to need a form with name, address, zipcode, city, state and comments fields integrated to client website and data inserted into MS SQL database. Do well on this project and I will have many more projects for you later. Have more work that we can handle.

server! If PHP/ASP/CGI is used, must install and configure on Windows 2003 server. You will have ftp and remote desktop access to the server. MUST BE ABLE TO EDIT FLASH... The contact form has 3 fields: name, e-mail addy and message. I need this done ASAP. Whoever can turn this one around quickly will receive additional work soon. Must be able to 100% intergrate. I've wasted too much time with this today I need this turned around in a couple of hours!
