Create a working form in Flex:
The form will be a working version of this:
http://ensurall.com/test/Form2.html
The Flex form will be a version of this html form - which submits the data to salesforce:
http://ensurall.com/test/index.htm
The Vehicle information page will be filled with data supplied in some CSV files.
The vehicle fields are dependent on each other:
Trim depends on Vehicle Model
Vehicle Model depends on Vehicle Make
Vehicle Make depends on the Vehicle Year
The form field ID's must be the same - but the id format causes problems in Flex, so some processing maybe required.
After the form is filled out - the data should be sent to salesforce.

I currently have one however I need to change it as I have added a few lists ton my a weber account and need thee lists to appear on this web form so the correct subscriber goes to the correct list.

Autopopulate Pdf Fields Using Data From Web Form.

3) The PDFs contain the name, address, phone info from the web form, and the user can fill out any missing data using the PDFs editable fields.
The PDFS I have are all already editable. I just don't know how to connect a form to those fields in the PDF. Please Note: Most fields will be repeated multiple times, so for example, if you supply your name, it will be applied to each PDF that contains a name field.

Web Form Calculations
The attached spreadsheet is the design for a web form to allow users to enter values and have the page update results based on the formulas in the spreadsheet. The programmer will need to create the web form and the formulas need to be hidden (not visible in a View>Source command) using server-side script or other means to do the actual calculations.
Zip Code Search For Web Form
I need a zip code search based on city that will allow user to search for a zip code and select value to insert into the "Zip Code" field in a web form. The search would pop up in a thickbox/lightbox window (can use jquery if needed) and after finding the zip code, selecting it would insert the value into the form field.
