Migration from Microsoft CRM/Dynamics Online to on-premise

We are looking to migrate our existing Microsoft CRM/Dynamics Online 4.0 data to our own-premise Microsoft CRM Online 4.0 server.
Microsoft has a policy to only perform online migrations after 12 months of service. Since we are Microsoft Certified Partner and we already have installed CRM locally, we don't want to wait and to pay extra months that we don't need. The data set is relatively small since we have less than 5 users, only 2 of which have used the CRM regularly for the past 2 months. I'm sure a MSCRM expert can get this done in a couple of hours max.
To complete a successful migration the provider must:
1)Export existing CRM data (e.g. to CSV format) from the online Microsoft Dynamics site. I found some tools to do that, but I'm not an expert in Microsoft CRM so I couldn't come up with a full pipeline that would preserve all or most of my data.
2)Re-create a few custom fields and transfer our data to our on-premise system, either through the CRM interface, or through MS SQL Server (we'll open ports and/or provide remote desktop capability if necessary)
Successful project completion will mean
1)All data is available in the new CRM environment including custom fields, attached files, and history records
2)CRM forms / views have been customized to the same specifications as the existing environment (note: we did very few of these)
3)All record relationships are maintained and valid.
I'm willing to pay up to $ 250 if this is performed in the next few days by an experienced CRM consultant.
