Graphic Design and Logo design Forms - clients to fill out

I am looking for someone who can help with a complete set of forms for Graphic Design and Logo design services.
Initially, I have in mind 2 forms needed.
1. A form for clients to fill out when they need a Graphic Design Service.
2. A form for clients to fill out and write what they want to have their company's logo designed. Logo Design Service.
The two forms must contain all the questions and details of what the clients would need and what the clients would provide as preference and other infos that we can collect from the clients. The two forms ate something that I can think of and would like to create.
I would need someone who know about the theory for this in order to create such forms (just in MS Word format) with boxes that we can check or cross (clients will fill out) and then clients can write their own comments, etc.
If you have other ideas and experience in doing the forms before, please feel free to share your suggestions in PM and tell me how much you would charge to create them and how long does it take to complete the two forms.

I have designed a number of logos and attached them. The logo idea of red and balck is that Commercial Guardian helps business go from in the red to in the black. The logo does not have to be red and black. The main logo should be Commercial Guardian - with the tag line of "improving your bottom line" as per the attached word document.

Already have the basic design of the logo, just require design and technical services. If happy with the work, there will also be design required for business cards.

I have a new Geodesic solutioons ClassAuctions Enterprise Edition v6 script, I need the website designed and the logo designed and uploaded onto the site. I'm a software engineer looking for a personal website and logo design. Simple, clean, and modern. The site will contain a short bio as well as links to my social networking sites.

They are very important for my branding strategy so they must look very professional and clean.
