Travel website private affiliate branding. Need logo to be designed, header, footer page integration

Very simple straight forward project. This is for the private branding of an affiliate networks travel page. What we need is a header page, footer page, as well as a logo to be designed, thats pretty much it!
Specifics in its entirety:
Basically, the header/footer pages have to flow with the affiliates pages which will be located in the center of the actual viewable website, with our branding on the top and bottom. You will be provided with full access to the account in order to accomplish this, by setting it up to work correctly. What we need is for the whole website to flow, without any indication of external affiliate pages being displayed. In other words, make it seem independent by link masking if possible to hide links (code in header/footer to acheive this?), as well as look consistent throughout all pages for example: cruises, flights, car rentals etc. We want it to seem as though the user has always been at the same site (our site).
Here is an example of a website utilizing a header and footer in order to match with all the pages.

This should give you an idea of what is needed.
The code you create will be pasted directly into the affiliates website, including the logo which will be stored on our own private server which you will be given access to (if you can do that with the header/footer pages to work around their pasting code system, meaning store the pages on our server and link to them by code on their site, thats acceptable as well). All you are required to do, is upload it there, and link to it with code in order to display. We need you to make sure that the pages you create, render correctly in all browsers, ie: firefox, ie, chrome, opera, safari to name a few.

Logo to be designed for a new moving company
