Scan 100-pg manuscript into Word Doc and correct errors

Request for Quotation to scan a 100-page single spaced printed Word document back into a Word for Windows 2003/2007 or the OpenOffice.org equivalent Word Processing compatible software as a disk file (.DOC or .DOCX file extension) in Microsoft Windows format to match the original manuscript exactly.
Note: I lost the Word Doc disk file for a printed manuscript done in Word for Windows and would like you to estimate what it would cost to scan each individual laser printed page out of 100 pages which ontain about 700 words per page - single spaced. The spot where the spiral binding intercepts the page in most cases caused 1-2 letters per line to be obliterated and thus not scannable.
This should be factored into your quotation. At about 45-50 lines per page time 1 or 2 missing letters per line that would be about 5,000 lines containing about 5,000 to 10,000 missing unscannable letters which would have to be manually corrected based on context. Proper names and places at the end of a line would not be expected to be perfectly corrected.
Then I need you to take the OCR scanned document and proof read and correct any of the words and footnotes which were not scanned correctly by the OCR scanner by visually comparing the scanned OCR first cut result with the actual printed hard copy.
I can send you a few sample pages in hard copy if that would be
helpful to you. The total appears to be about 70,000 words counting
all 100 pages. The Index was auto-generated using Word and would
not have to be typed again.
