Survey is 13 questions, radio buttons allowing 1 answer per question
After survey is completed, user is taken to a results page (based on answers to questions)
The formula for results will be supplied to you in excel format (when coded on website this should formula should not visible as part of the source code)

Once each survey is complete it needs to be forwarded to the individual users login, or email address. This project will be ongoing, and we are looking for a skilled programmer who can make changes to the system as needed. Install Pop-up Exit Survey On Website
should not take you more than 30 minutes - 1 hour to implement because I have the code, I just need someone to write a quick JavaScript pop-up script. I would prefer that the script for the pop-up not be coded on the HTML page itself, but be called.

3) Remove current advertising banner and add new December promo banner (you will receive PSD file for the banner)

Css Install On Website

Simple copy and paste job, I don't know Css. Once it's done, I will need to input my sales copy in the site.

e mouse across all tabs when page was loading and all tabs say “HOME”

This happens to all page if the user runs their mouse across flash tab menu when page is loading – this is the only thing I need fixed but I don't know flash so I don't know how to fix

For the second stage, my intent is to use PDF expert to collect other information and sign and send flattened copies to both customer and office. I can do this on my own however if there is a simple, AFFORDABLE, and better solution I am open to it.
